Enrolled or Opt In
Your Benefits Are Active — or On the Way
You’re participating in the Oaceus 360 Benefits Program through your employer-sponsored plan. It’s designed so you keep the same take-home pay while tax savings fund the medical benefits; actual results vary, and not all employees qualify. As long as you remain qualified and keep meeting the program’s participation requirements, your benefits stay active.
Opt In — How Is It Different?
If you recently chose to enroll, your status is Opt In while we complete a short qualification check — the step between your election and active benefits. It confirms you generate enough tax-withholding savings to fund the plan. We check this before enrolling you because the goal is access to preventive care — not a smaller paycheck or a surprise tax bill at year-end. Once the check clears, your enrollment activates and your status becomes Enrolled.

Our records show you elected to opt out during your initial 30-day enrollment period. Because that election was made during your enrollment window, it remains in effect once your Opt-Out form is received and processed.

Changed Your Mind? How to Opt In
You can opt in after previously opting out — no Qualifying Life Event needed. Here’s the process:
- Contact your Oaceus Benefits Specialist — we’ll review your current qualification status and explain the steps required to enroll.
- Complete the SRA Opt-In form and acknowledge how the change may affect your payroll and taxes.
- Once your documentation is complete and your qualification is confirmed, Oaceus can begin the enrollment process.
We encourage you to meet with a Benefits Specialist to talk through your options.
Qualifying Life Event (QLE)
Need to Make a Change to Your Benefits?
After your initial 30-day enrollment window closes, changes to your Oaceus 360 benefits generally require a Qualifying Life Event (QLE). The Oaceus 360 program is administered through your employer’s Section 125 cafeteria plan, and IRS regulations limit when benefit elections can change outside your initial enrollment period. If you want to opt out, opt in, or request another eligible enrollment change after your effective date, you’ll need to submit a Qualifying Life Event request.
What Is a Qualifying Life Event?
A Qualifying Life Event is a significant life change recognized under IRS regulations that lets you change your benefit elections outside your initial enrollment period.
Family Changes
- Marriage
- Divorce or legal separation
- Birth or adoption of a child
- Death of a spouse or dependent
Employment Changes
- Change in employment status
- Significant change in work hours
- Beginning or ending employment
- Returning from a leave of absence
- Changes in major medical coverage
What Changes Can I Request?
- Opt out of the Oaceus 360 program after the 30-day enrollment period (mid-year)
- Request enrollment (Opt In)
- Reinstate participation if you were previously disqualified (DNQ)
- Update your benefit elections, when the plan permits
All requests are reviewed for compliance with plan rules and IRS regulations.
What Documentation Will I Need?
To process your request, you may need to provide documentation supporting your Qualifying Life Event, such as:
- Marriage certificate
- Divorce decree
- Birth certificate
- Adoption paperwork
- Proof of other insurance coverage
- Employment documentation
- Other supporting records
If documentation isn’t available, your employer can complete an HR Verification Form to verify the event.
Important Timing Requirements
Submit your Qualifying Life Event request as soon as possible after the event occurs, and within 60 days. Requests submitted outside your employer’s allowable timeframe may not be eligible for processing until the next enrollment opportunity.
Frequently Asked Questions
Why can’t I just opt out after 30 days?
Once your initial enrollment window closes, IRS rules governing employer-sponsored Section 125 benefit plans generally require a Qualifying Life Event before you can change your benefit elections.
Can I opt back in later?
Yes. If you experience a Qualifying Life Event, or become eligible during your employer’s next enrollment opportunity (assessed every 30 days), you can request to enroll, subject to plan qualification requirements.

Terminated Employment
Your Policy Can Stay With You
Our records show your employment with your participating employer has ended. Your employer-sponsored participation ends with it — but you may still have options.
Universal Life Insurance
If you were enrolled in the Transamerica Universal Life Insurance policy, you own it. Transamerica will contact you about continuing the policy by paying premiums directly. Your options:
- Disregard notice — let the policy lapse
- Cash value — meet with a Transamerica representative to see if there’s a surrender value
- Flex premium — request a change in premium amount
- Direct pay — set up premium payments directly with the carrier
If you choose not to keep paying premiums, the policy will eventually lapse according to its terms.
Did Not Qualify (DNQ)
Not Qualified Today — There's a Path Back
Our records show you don’t currently meet the qualification requirements for the employer-funded portion of the Oaceus 360 program. This most often happens when there isn’t enough federal tax withholding available to fund the plan without reducing your paycheck. The program is designed to avoid enrolling anyone in benefits that would create an unexpected tax liability or shrink their take-home pay — and not every employee generates enough withholding savings to qualify. That’s what this status reflects.

What Does This Mean?
You’re not currently participating in the employer-funded version of the Oaceus 360 program. If you were previously enrolled, your Universal Life policy remains yours to own, and depending on your circumstances, you may be able to continue it directly with Transamerica.
Can I Become Qualified Again?
Yes. If your payroll circumstances change, you may become qualified in the future — for example, through increased taxable earnings, changes to tax withholding, payroll adjustments, or employer payroll changes. If becoming qualified would reduce your net pay, we’ll get your consent before enrolling you.
Want to Continue Benefits Anyway?
You may be able to purchase benefits directly, set up through payroll for an amount you agree to. If you generate any tax-withholding savings, those savings can be applied toward the premium and may reduce what you pay directly; actual savings vary and aren’t guaranteed.
